一、本岩主神清水祖師，源由宗親吳大乾之來台開基祖先吳氏宗長用米籃裝鹽，鹽內藏祖師神尊，先祖將鹽及祖師神尊擔自福建泉州安溪縣南安渡海來台，迎奉為保護神，庇佑信眾一路平安抵達，定居秀水庄，先祖吳氏宗長一同，恭奉清水祖師香火傳承。
二、清水祖師神尊因年久神尊表皮剝落，神尊身變黑，經信眾吳傳博、吳大乾等籌劃將清水祖師神尊送佛店整修，後開光點眼，回復原清水祖師神尊面貌，繼續供信眾膜拜，清水祖香火鼎盛救世萬民。
三、秀水岩恭奉清水祖師為主神，本岩供奉諸神（太子元帥、觀音佛祖、吳府千歲、萬教帝君、濟公活佛、福德正神）祈願諸神護本庄合境平安，俾為信眾祈安納福聖地，為本庄信仰中心，清水祖師香火鼎盛，永續傳承。
四、清水祖師神威顯赫救世萬民，本庄信眾吳芳洲善德，神威感應之下，在外經商有成，於民國78年春季無償捐獻土地兩筆，供秀水岩清水祖師廟地使用，當時經費有限沒對外墓款。
五、廟宇現用鐵皮構造至今，99年成立管理委員會，經彰化縣政府核准，經開信徒大會三分之二以上同意，101年申請重建新廟宇中，期待廟宇重建早日完成，香火永續傳承。
(Xiushui Rock-Qingshui Patriarch)
1. The ancestor of the main rock, Qingshui, originated from his clan, Wu Dagan, who came to Taiwan to open the foundation. The ancestor Wu’s ancestor filled the salt with rice baskets, and the ancestors carried the salt and the ancestors from Nan’an, Anxi County, Quanzhou, Fujian. Taiwan, welcoming as a protector god, bless the believers to arrive safely, settle in Xiushui Village, together with the ancestor Wu clan, and respect the inheritance of the incense from Qingshui Patriarch.
2. The deity of Qingshui Patriarch was peeled off over time, and the body of the deity turned black. According to Wu Chuanbo and Wu Daqian, the believers sent the Qingshui Patriarch to the Buddha shop for renovation, and then opened the eyes to restore the original Qingshui Patriarch’s face. Continue to worship the believers, the Qingshui ancestor incense flourishes to save the world.
3. Xiushuiyan respects Qingshui Patriarch as the main deity, and this rock enshrines the gods (prince marshal, Guanyin Buddha, the thousand years of Wu's house, Wanjiao emperor, Jigong living Buddha, Fude Zhengshen) praying that the gods will protect the village in peace and harmony. Believers pray for happiness in the holy place, which is the faith center of the village.
4. The ancestor of Qingshui has a great power to save the world, and the believers of this company, Wu Fangzhou, is good. Under the influence of God’s power, he made a successful foreign business. In the spring of 1978, he donated two land for free for the use of the temple of the Qingshui ancestor in Xiushuiyan. Funds were limited at that time. Outer tomb money.
5. The temple is currently constructed with iron sheets. The management committee was established in 1999, approved by the Changhua County government, and approved by more than two-thirds of the opening believers' assembly. In 101, the application for reconstruction of the new temple is expected to be completed soon and the incense will continue to be passed on.
