崙仔頂清水宮創建於日治明治30年(1995)，主祀清水祖師，奉祀神明有三官大帝、中壇元帥、福德正神等。現今廟貌則為民國88年(1999)重修。
 Establishment history:
The Lunziding Qingshui Palace was founded in the 30th year of the Japanese rule of Meiji (1995). It is dedicated to the ancestor of Qingshui and enshrines the emperor of the three officials, the marshal of the altar, and the god of merit. The current appearance of the temple was rebuilt in 1999.
