沙鹿古稱沙轆,原本是平埔拍瀑拉族 (Papora) 沙轆社的居住地,依據荷蘭人在西元1650年所做的人口調查,共有三十餘戶,計一百餘人,是平埔族中的大村落。根據沙鹿鎮誌記載,清乾隆初期,客家人吳瓊華入墾沙轆,為處理開墾戶的事情而建的寮舍,稱為「公館」,並迎請家鄉三山國王神尊,來台興建了屬於自己群族的客家廟宇「三山國王廟」。 根據保安宮廟誌記載,保安宮始建於乾隆十年 (1745),歷經多次的修葺重建,與福利宮交換位置的遷移,到民國73年 (1991) 的最後一次重建,迄今 (2020) 已有275年的歷史。 當年 (清乾隆年間) 福建同安縣的蔡姓,安西縣的王姓,南安縣的李姓及晉江縣的蕭姓等家族,也陸續到沙轆拓墾,「沙轆新庄」漸漸形成。至嘉慶年間沙轆已發展為南北交通必經之地。設有塘兵房三間,駐防守兵十五名,道光、咸豐年間,晉江縣的顏等姓家族、安溪的白姓家族,不斷的拓墾,漢人勢力迅速擴張,原本沙轆社平埔族人則遷往埔里,少數遷到「噶瑪蘭」(現今宜蘭),沙轆成為漢族客閩混居的聚落街庄。 沙轆客閩混居時期,因生活習俗的不同及利益糾葛,閩客衝突持續擴大與緊張.最後客家人不敵退走台中市東勢區.遺留下來的三山國王廟因乏人管理,又久經風霜破爛不堪,河洛人 (閩南人) 鑑於三山國王神靈顯赫及籤詩靈驗,並且不忍心見神像無處安身,乃踴躍樂捐進行改建.並取其 「保國安民」之意改名為「保安宮」。
